Parental Consent Form

Photography, Book, Media and Website(s)
Because we care deeply for children we want to be careful that each child's parent(s) sign a consent form and understand our projects purpose. The reason this is necessary is because we do photograph the children while they are working and would like to have a class photo taken if possible. Images are sometimes put on our website to share with others and can be published in the local papers. We do not publish last names or addresses. If you do not wish to have your child photographed they can still enjoy this wonderful project!
 

Kids Share Workshops celebrates all students of any race, gender, color, religion, national or ethnic origin, and does not discriminate. We believe all children are a gift and a reason to celebrate!
